“Sensitive Personal Information” means Personal Information that reveals a consumer’s (A) social security, driver’s license, state identification card, or passport number, (B) account log-in, financial account, debit card, or credit card number in combination with any required security or access code, password, or credentials allowing access to an account, (C)  precise geolocation, (D) racial or ethnic origin, citizenship or immigration status, religious or philosophical beliefs, or union membership, (E) mail, email, and text messages content unless we are the intended recipient of the communication, (F) Personal Information collected and analyzed concerning a consumer’s health, (G) Personal Information collected and analyzed concerning a consumer’s sex life or sexual orientation.

Children Under The Age of 13

Our Website is not intended for children under 13years of age. No one under age 13 may provide any Personal Information to or on the Website. We do not knowingly collect Personal Information from children under 13. If you are under 13, do not use or provide any Personal Information on this Website or through any of its features, register on the Website, or provide any Personal Information about yourself to us, including your name, address, telephone number, email address, or any screen name or user name you may use. If we learn we have collected or received Personal Information from a child under 13 without verification of parental consent, we will delete that information.   • Geolocation Data. This includes Personal Information that discloses your physical location or general location.
     

  • Professional or Employment-Related Information. This includes Personal Information such as where you work or what job you perform.
     

  • Inferences. This includes Personal Information drawn from any of the above categories to create a profile about a user, which reflects their preferences, characteristics, psychological trends, predispositions, behavior, attitudes, intelligence, abilities, and aptitudes.
     

  • Sensitive Personal Information. This includes Personal Information such as account access information in combination with financial information, your precise geolocation, your racial or ethnic origin, your genetic data, or data regarding your health.

 

We collect this Personal Information:
 

  • Directly from you when you provide it to us. This includes Personal Information entered into our Website at the time of registration or other continued use and records and copies of correspondence between you and the Company.
     

  • Automatically as you navigate through the site. Information collected automatically may include usage details, IP addresses, and information collected through cookies or web beacons.
     

    • Cookies (or browser cookies). A cookie is a small file placed on the hard drive of your computer. You may refuse to accept browser cookies by activating the appropriate setting on your browser. Why We Collect Your Personal Information
 

We use Personal Information that we collect about you or that you provide to us to provide concierge support services to you during your cancer journey. We will use your Personal Information to help us advocate for you, explain complex medical concepts and new research, communicate with your health care providers and care team, and schedule appointments. For example, we may use your (i) postal address to connect you with support groups near you or (ii) insurance policy number when we help you find in-network providers and set up appointments.
 

We also use your Personal Information for the following purposes:
 

  • To present our Website and its contents to you.
     

  • To provide you with notices about your account or subscription, including expiration and renewal notices.
     

  • To carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collection.
     

  • To notify you about changes to our Website or any products or services we offer or provide though it.
     

  • In any other way we may describe when you provide the Personal Information.
     

  • For any other purpose with your consent.

 

We will retain your Personal Information for the duration of your use of our services or for any period of time required by law.

We may disclose aggregated information about you, and other information that does not identify you, without restriction.
 

In the preceding 12 months, we have disclosed the above listed categories of Personal Information:
 

  • To health care providers and care team, insurance companies, support and advocacy groups, non-medical hospital personnel, and other third parties that a customer requests.
     

  • To contractors, service providers, and other third parties we use to support our business and who are bound by contractual obligations to keep Personal Information confidential and use it only for the purposes for which we disclose it to them.
     

We may also disclose your Personal Information:
 

  • To a buyer or other successor in the event of a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of Hippocrates’ assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which Personal Information held by Hippocrates about our Website users is among the assets transferred.
     

  • To comply with any court order, law, or legal process, including to respond to any government or regulatory request.
     

  • To enforce or apply our Terms of Use and other agreements, including for billing and collection purposes.
     

  • If we believe disclosure is necessary or appropriate to protect the rights, property, or safety of Hippocrates, our customers, or others. This includes exchanging information with other companies and organizations for the purposes of fraud protection and credit risk reduction.

 

Does Hippocrates Sell or Share My Personal Information?

 

No, Hippocrates does not Sell or Share Personal Information. Additionally, Hippocrates does not have actual knowledge that it Sells or Shares the Personal Information of consumers under sixteen (16) years of age.

 

“Selling” means selling, renting, releasing, disclosing, disseminating, making available, transferring, or otherwise communicating your Personal Information to a third party in exchange for money or other compensation.

 

However, Selling does not include:
 

  • Disclosures or interactions with third parties as directed by you.
     

  • Transfers to processors that process Personal Information on our behalf.
     

  • Disclosures necessary for providing Hippocrates’s services that you request.
     

  • Transfers to affiliates our affiliates.
     

  • Disclosures of Personal Information made publicly available by you.
     

  • Transfers as part of a merger, acquisition, or similar transaction, provided Personal Information is used consistently with the original terms.
     

“Sharing” means selling, renting, releasing, disclosing, disseminating, making available, transferring, or otherwise communicating a consumer’s Personal Information to a third party for Cross-Context Behavioral Advertising, even if no money is exchanged. “Cross-Context Behavioral Advertising” means the targeting of advertising to you based on your Personal Information obtained from your activity across businesses, distinctly branded internet websites, applications, or services, other than such activity on Hippocrates’s platforms.

 

However, Sharing does not include:
 

  • Disclosures or interactions with third parties as directed by you.
     

  • Alerting other third-parties that you have opted out of sharing your Personal Information.
     

  • Transfers as part of a merger, acquisition, or similar transaction, provided Personal Information is used consistently with the original terms.

Your Privacy Rights

Right to Delete

 

You may request that Hippocrates delete the Personal Information we collected about you. If your request is accepted, Hippocrates will delete your Personal Information from our records, and we will notify any service providers, contractors, or other parties with whom we have shared or sold your Personal Information to do the same.

 

We may retain a confidential record of your deletion request to prevent the Personal Information from being sold or to comply with any applicable laws.

 

Right to Correct

 

You may request that Hippocrates correct inaccurate Personal Information. We will use commercially reasonable methods to comply with your request.

 

Right to Know What Personal Information is Being Collected/Right to Access Personal Information

 

You may request that Hippocrates disclose (1) the categories of Personal Information we collect about you, (2) the sources from which we collect the Personal Information, (3) the business or commercial purpose for collecting Personal Information, (4) the categories of third parties to whom we disclose Personal Information and (5) the specific pieces of Personal Information we have collected about you.

 

Right to Limit Use of Sensitive Personal Information and Notice of Right to Limit 

 

You may request that Hippocrates limit our use of your Sensitive Personal Information to what is necessary to perform our services.

 

If you want to limit our use of your Sensitive Personal Information you or your authorized agent can text us at 650-283-1414 or follow this link. We will comply with your request and notify any relevant third parties of your request within 15 business days from the date we receive your request.

 

Right to Non-Discrimination

 

Hippocrates respects your right to privacy. We will not discriminate against you because you have exercised any of your rights under this Privacy Policy. Specifically, we will not deny you access to our services, charge you different prices, or provide a different level of quality of services if you exercise your privacy rights.2

How Can I Exercise My Privacy Rights?

Upon receiving your request, we will need to verify your identity. These verification efforts require us to ask you to provide information so that we can match it with information you have previously provided us. For instance, depending on the type of request you submit, we may ask you to provide certain information so that we can match the information you provide with the information we already have on file, or we may contact you through a communication method (e.g., phone or email) that you have previously provided to us. We may also use other verification methods as the circumstances dictate.

 

We will only use Personal Information provided in your request to verify your identity or authority to make the request. To the extent possible, we will avoid requesting additional information from you for the purposes of verification. However, if we cannot verify your identity from the information already maintained by us, we may request that you provide additional information for the purposes of verifying your identity and for security or fraud-prevention purposes. We will delete such additionally provided information as soon as we finish verifying you.

 

An authorized agent, such as a parent, guardian, or conservator, may submit your request on your behalf, however, we will have to similarly verify the authorized agent’s authority to submit such request.

 

We will respond to your request within 45 days, or we will notify you if we need to extend our review period for an additional 45 days.

 

Data Security
 

We have implemented measures designed to secure your Personal Information from accidental loss and from unauthorized access, use, alteration, and disclosure. [All information you provide to us is stored on our secure servers behind firewalls. Any payment transactions will be encrypted.]
